Thanks for the reply. Problem is solved now by fresh installation. I gave 127.0.0.1 as the database address (instead of local host) during installation and things went as expected.

I have just installed formalms 1.4.2 on xampp 5.5.38 on my windows 7 machine. Step 7 of the says installation completed and there are some more details on the page.
