Not able to create users in 3.1

New to FORMA LMS and evaluating 3.1 version ..As part of installation created Super-Administrator user but not able to create any more users . when i hit submit it just gives an '!' with no error . Any help is greatly appreciated.

I can't replicate your issue on a 3.1.
There must be something wrong with your environment.
You can contact me in private if you need a debugger on your instance.
