unlimited use of subscription code,
the uses counter in code tab didn't rises.

I don't understand the supposed bug you are highlighting. Can you describe more precisely what you are trying to achieve and what happens instead?

By the way, did you update to Forma 1.3? I don't know if this is the problem you are facing, but if you take a look at the changelog here http://formalms.org/versions/changelog.txt you'll see that the subscription by code is the first bugfix listed.
