Hello everyone,

I have just installed Forma LMS and successfully completed the installation process. However, when I try to access the system, it takes me directly to the login page. I haven’t created any users yet, so I’m attempting to log in with the default "admin" user.

Unfortunately, I’m encountering the following errors:

PHP Warning: Undefined array key "" in C:\inetpub\wwwroot\FormalMS\lib\lib.usermanager.php on line 1068
PHP Notice: Only variable references should be returned by reference in C:\inetpub\wwwroot\FormalMS\lib\FormaUser.php on line 610

I am not sure what is causing these errors, and I haven't been able to proceed beyond this point. I would appreciate any guidance or suggestions on how to resolve this issue.

We are using IIS (the equivalent of Apache on Linux), PHP 8.1.29, MariaDB 10.5.26 and forma 4.0.0

I think IIS - though it's very distant to be "equivalent" to Apache - is not the problem.
Forma 4 it's said to be working with mariadb <= 10.3, so the first thing to do is downgrading mysql server.
If this won't be of help, turn completely off all errors and log the fatal ones on the file system. Then try again, if you have a blank page, post the fatal.

I'm trying to download it from Linux now, but when I get to the part to define the database, a message appears with "Sql Server Version unsuitable to requirements", but the address I'm using is localhost, do you know how I can proceed?

What does localhost have to do with mysql version?
The message is clear. What mysql server package did you install?
