first you have to define the additional fields that you want (tel, country...)
then you assign them to the organization chart. If you wish to, add them to the root of the org chart. You can also decide if they have to be mandatory.
Now, if you set self-registration (I think you already set it), your registration form should include all the additional fields.

The extra fields will be automatically added to the form once you enable them on the organization chart.
There is a small icon beside each node allowing you to define wich fields will be assigned, mandatory and visibile or not to the user. If you do this for the root node this will be applied to all the others, and the fields will be visible in the registration form.

administration>main>configuration>settings>user
scroll down and you will find the "register" settings
you can choose among:
Free self registration (the user self register and that's all)
moderated self registration (admin has to approve)
only admin

But did you create the new additional fields?
Did you assign them to the "root" folder of the org chart, and all descending subfolder?

Username, email, first name and last name are not additional fields. They are the default fields and are already there, no need to add them.
Same thing for password and retype the password.
so, you just need to add 3 additional fields (administration>main>configuration>additional fields): tel, city, country
then, as Alberto explained, assign them to the root folder of the org chart
