Instructor Rights
I have assigned an instructor role, and have made sure the view/edit access is given to instructor in the menu; however, when the instructor logs in, he can see the gradebook; can add activities; but cannot see the students in the course - only himself. Any fix?

You should reassign menu to course after each modification.

I have done that. The instructor still cannot see the students, only himeself in the grade book. Also, I noticed that the only way the instructor can see all students in the gradebook is only when I edit him from 'user' to 'super administrator' - but I don't want to do that - Instructor should be able to see all students. Thanks for that reply, but that didn't fix my issue. I even created another menu, assigned that to the course; and still same.

Let me explain why the gradebook (and similarly all stats and teacher modules should behave that way) is behaving as you describe.
When a user (a simple user, not an administrator) is subscribed to a course with teacher level (or tutor, or any level allowed to see gradebook module according to your menu permissions), we may want he to see all of the students in his course.
But, we may not.
Why is very simple.
Let's make an example. We have many users in our LMS, divided into groups (or org chart nodes, it's the same).
Each group has its own admin (I mean not simple users, but Administrators), who can see only the users who are in the groups he/she administers.
We have a single course, where all users are subscribed as students.
Do you want admins of group A can see students even if they are users of group B? Privacy issues, conflicts of interests, info shared among competitors... I see many possible problems in such an approach.
If you don't mind, you can make some adjustments to the code of the gradebook module.
This will work for the whole platform, across all users and courses.
